Job summary
Oregon Health & Science University (OHSU) in Portland, OR is seeking a Heart Failure Nurse Coordinator to oversee comprehensive care for advanced heart failure patients across inpatient and outpatient settings.
The role entails coordinating education, leading clinical staff, and partnering with physicians, nurses, and community services to optimize transitions of care and patient outcomes. Strong cardiovascular nursing background and licensed RN status are essential.
Qualifications
- Minimum of 3 years recent cardiovascular nursing experience.
- Current, unencumbered Oregon State Registered Nurse License.
- Washington license within 6 months of hire.
- BLS from AHA required; orientation will include training.
- Must enroll in BSN if ADN holder within 3 years.
Responsibilities
- Coordinate inpatient and outpatient heart failure patient care.
- Provide staff education and nursing competencies.
- Collaborate with care team and community partners for transitions of care.
- Support guideline-directed heart failure care and quality improvement.
- Discharge planning and follow-up coordination.
- Engage in patient/family education on disease management.
