Providence is seeking an RN Coordinator in Portland, OR to support our heart program. The role focuses on coordinating patient contact, screening, and initiation of care while aligning with the Providence heart network.

The position requires a Bachelor of Science in Nursing, Oregon RN licensure, and 5 years of clinical cardiac experience. We offer a collaborative environment with strong benefits and a commitment to patient-centered care.

Formación

• Bachelor's Degree in Nursing.
• Oregon RN License at hire.
• National Provider BLS - American Heart Association.
• 5 years of clinical experience in acute care, cardiac ambulatory and/or cardiac interventions.

Responsabilidades

• Coordinate patient contact and screening evaluations within heart program.
• Liaise with referring providers and hospitals within Providence heart program.
• Ensure patient progression and care delivery across the continuum in collaboration with clinical leaders.
