Qualifications

Responsibilities

• Manage patient flow, triage and direct patient care.
• Liaise between front desk, nursing staff, physicians, and clinic manager.
• Support patient self-management and disease education.
• Coordinate continuity of care with hospital admissions, discharges, and ER visits.
• Document assessments and plans in EMR and communicate results.
