Job summary

HELP of Southern Nevada is seeking a New Beginnings Intensive Case Manager to provide direct intensive case management for chronically unhoused adults and to help move clients into long‑term housing. You will conduct intake, build individualized case plans, review progress, and connect clients with essential services and resources through HELP and partner agencies.

The role requires collaboration with the AHP Supervisor, data entry in HMIS, and maintaining audit‑ready files while upholding

Qualifications

  • Proof of Highschool Graduation or GED
  • 2 years' experience with data entry, compliance, or related field
  • Previous experience working with unhoused population

Responsibilities

  • Under the direct supervision of the AHP Supervisor, the New Beginnings Intensive Case Manager provides direct intensive case management services to chronically unhoused adults.
  • Conduct initial interview/intake and client needs assessments
  • Provide case management and supportive services to clients moving into long‑term housing
  • Create individualized case plans with each adult member of the household, based on the client's needs
  • Review the client's treatment plan monthly, update the client's case plan, and track outcomes
  • Connect clients with wrap‑around services through other programs offered at HELP, including Workforce Services Job Readiness Workshops, Energy Assistance Program, holiday programs, etc.
  • Connect clients with mainstream programs including but not limited to; Medicare, SSI, and SSDI
  • Provide clients with referrals and resources for other social services and partner agencies
  • Meet with clients to complete income calculations and rent requests regularly
  • Conduct weekly and semi‑monthly home visits
  • Provide transportation to clients to for appointments
  • Enter case notes into HMIS daily, ensuring captivity of client's progress
  • Maintain audit‑ready files
  • Maintain clear boundaries with all clients and providers ensuring ethical standards are not compromised
  • Facilitate Life Skills Training sessions for clients as needed
  • All other related duties as assigned by management for the needs of HELP of Southern Nevada

About the Department

The New Beginnings Intensive Case Manager (ICM) provides permanent supportive housing to chronically unhoused adults without children (AWOC). Support participants in overcoming challenges and building a pathway to a better quality of life, foster self‑sufficiency, and stability.

Job Summary

The ideal candidate will possess the following knowledge, skills, and abilities
  • Knowledge of local social service agencies, programs, and issues as they pertain to the local unhoused population
  • Knowledge of applicable statutes, ordinances, and codes
  • Knowledge of social service work, interviewing, and referral techniques
  • Ability to analyze situations, exercise appropriate judgement, and adopt an effective course of action
  • Ability to communicate effectively with persons from a variety of social, cultural, and economic backgrounds, and possess a demonstrated understanding of and sensitivity to culturally diverse populations
  • Demonstrate strong written, verbal, and listening skills, along with the ability to independently initiate and respond to various forms of correspondence to all business contacts
  • Ability to be flexible, adjust to shifting priorities while remaining organized
  • Ability to understand and follow all agency policies and procedures
  • General computer knowledge, and the ability to use Microsoft Office software
  • Ability to work flexible hours, as required by the client caseload
  • Ability to perform accurate mathematical equations, calculate client's rent/ utility costs
Required Qualifications

  • Bachelor's Degree in Social Work or related field
