Job summary

Home & Care Home Health and Everwind Hospice is seeking a Community Relations Director to build and maintain referral partnerships with healthcare providers, facilities, and community organizations. This role drives growth, coordinates education events, and collaborates with clinical teams to ensure timely admissions.

The position requires strong relationship-building, healthcare sales experience, and a willingness to travel within the territory.

Qualifications

• Bachelor’s degree preferred; equivalent healthcare sales experience considered.
• Minimum of two years of healthcare sales, marketing, or business development experience preferred.
• Excellent communication and relationship-building skills.
• Strong organizational and time management abilities.
• Valid driver’s license with reliable transportation.
• Ability to travel throughout assigned territory.
• Must be able to successfully complete a fingerprint-based background check and pre-employment drug screening as a condition of employment, in accordance with applicable laws and regulations.
• Preferred: Home Health or Hospice sales experience.
• Knowledge of Medicare, Medicaid, and commercial insurance.
• Existing healthcare referral relationships.
• CRM experience.

Responsibilities

• Develop and maintain referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, discharge planners, and community partners.
• Execute weekly territory management plans to maximize referral growth.
• Conduct scheduled face-to-face sales calls each week.
• Develop new referral sources while strengthening existing partnerships.
• Represent the organization at networking events, health fairs, and community outreach activities.
• Coordinate educational presentations for referral partners and community organizations.
• Identify patients appropriate for Home Health and Hospice services.
• Collaborate with clinical teams to ensure timely admissions.
• Communicate with referral sources throughout the referral and admission process.
• Monitor referral pipelines and provide follow-up on pending referrals.
• Assist referral partners in navigating eligibility requirements and payer guidelines.
• Serve as the primary point of contact for assigned referral accounts.
• Resolve concerns quickly and professionally.
• Maintain regular communication with referral partners.
• Ensure an exceptional customer experience for patients, families, and referral sources.
• Promote Home & Care Home Health and Everwind Hospice services throughout the community.
• Coordinate physician offices, facility in-services, lunch-and-learns, and educational events.
• Assist with community marketing campaigns and social media initiatives.
• Participate in community outreach and volunteer opportunities.
• Attend weekly sales meetings.
• Participate in interdisciplinary meetings as appropriate.
• Work closely with clinical leadership to identify patients appropriate for Home Health or Hospice.
• Share market intelligence and competitor activity with leadership.
