Job summary

RiseMe is seeking a Community Manager to drive census growth and develop referral relationships across hospitals, senior living communities, and physician networks. You will serve as the market ambassador and manage end-to-end client onboarding with a focus on revenue growth.

You will conduct visits, presentations, and outreach, track KPIs, and maintain strong partnerships while representing RiseMe at industry events and within assigned territories.

Qualifications

• Bachelor's degree preferred.
• 3+ years healthcare marketing, business development, home care, hospice, senior living, or related experience.
• Strong networking and relationship-building skills.
• Excellent communication and presentation abilities.
• CRM proficiency preferred.
• Reliable transportation for community visits.

Responsibilities

• Build and maintain relationships with hospitals, rehabilitation facilities, senior living communities, physicians, social workers, discharge planners, hospices, and other referral sources.
• Conduct regular visits, presentations, and networking activities to generate referrals.
• Develop and execute territory growth plans.
• Track referral activity and conversion rates.
• Identify opportunities to expand services within current referral accounts.
• Represent the organization at networking events, community events, and professional associations.
• Coordinate educational presentations and in-service programs.
• Promote the company's services and value proposition.
• Maintain visibility within assigned territories and communities.
• Meet monthly targets for new client starts, revenue growth, referral source development, and qualified prospect generation.
• Collaborate with operations to ensure smooth client onboarding.
• Follow up with prospective clients and families.
• Maintain ongoing communication with referral partners.
• Address concerns and ensure referral partner satisfaction.
• Provide regular updates to leadership regarding market trends, opportunities, and threats.
