Hireology is seeking a Community Manager to drive census growth and referral networks in the Chicago area. You will develop partnerships with hospitals, senior living communities, and physicians while promoting services and delivering new client opportunities.

In this role, you will manage territory growth plans, track referrals, and ensure smooth onboarding with operations. Strong networking, presentation skills, and CRM proficiency are essential for success.

Qualifications

• Bachelor's degree preferred.
• 3+ years healthcare marketing, business development, home care, hospice, senior living, or related experience.
• Strong networking and relationship-building skills.
• Excellent communication and presentation abilities.
• CRM proficiency preferred.
• Reliable transportation for community visits.

Responsibilities

• Build and maintain relationships with hospitals, rehabilitation facilities, senior living communities, physicians, social workers, discharge planners, hospices, and other referral sources.
• Conduct regular visits, presentations, and networking activities to generate referrals.
• Develop and execute territory growth plans.
• Track referral activity and conversion rates.
• Identify opportunities to expand services within current referral accounts.
• Represent the organization at networking events, community events, and professional associations.
• Coordinate educational presentations and in-service programs.
• Promote the company's services and value proposition.
• Maintain visibility within assigned territories and communities.
• Meet monthly targets for new client starts, revenue growth, and referral development.
