Job summary

Care New England is seeking an Intake Customer Service representative in Warwick, RI to ensure intake information meets admission criteria and is processed accurately for the appropriate team.

You will contact referral sources, verify discharge reports, and coordinate with departments to service patients in a timely manner, while maintaining up-to-date records and utilizing MS Office.

Qualifications

• Education: High School diploma required; Associate degree preferred.
• Minimum 3 years in healthcare, call center experience preferred.
• Experience with managed care and medical terminology recommended.
• Strong computer and administrative skills; proficient in MS Office.

Responsibilities

• Respond to referral sources efficiently, effectively and pleasantly.
• Process all intake information accurately for the appropriate team.
• Verify patient discharge reports and contact other departments as needed.
• Check discharge for referrals and prepare admittances per schedule.
• Respond to log inquiries from community resources and private duty services.
• Answer phones, gather demographics, physician and insurance info, and forward referrals to RN/LPN or manager.
• Transcribe information into records, maintain E-Discharge and IARs.
