Job summary

Greystone in Monroe, NY is seeking a Health Care Coordinator to provide direct care and administrative support in a group home setting. The role combines Direct Support Professional duties with clinical coordination to the RN and Residence Manager.

Responsibilities include scheduling medical appointments, administering medications, and tracking health data, with AMAP certification required or obtainable within three months. Sunday–Thursday, 7am–3pm, full-time position.

Qualifications

• Adult development disability experience preferred, caring and respectful.
• AMAP certification required or able to obtain within 3 months.
• Operate in a group home setting with RN and Residence Manager.
