Job summary

Pointe Healthcare is seeking a Community Liaison to grow census and build referral partnerships across Oil City, Saegertown, and surrounding communities. You will engage healthcare providers, hospitals, and families to connect them with our skilled nursing and rehabilitation centers.

The role emphasizes relationship-building, territory travel, and cross-functional collaboration with Admissions and Leadership to ensure smooth patient transitions and strong brand presence in the community.

Qualifications

• Bachelor's degree preferred in Marketing, Business, Healthcare Administration or related field.
• Previous healthcare marketing, admissions, business development, sales or long-term care experience.
• Knowledge of skilled nursing, rehabilitation, or post-acute care is desirable.
• Excellent communication and relationship-building skills.
• Strong organizational and time management abilities.
• Ability to work independently while managing multiple priorities.
• Proficiency with Microsoft Office and CRM/database systems.
• Valid driver's license with reliable transportation.

Responsibilities

• Develop and maintain relationships with hospitals, physicians, discharge planners, case managers, social workers, and community organizations.
• Generate new referral sources while strengthening existing partnerships.
• Conduct regular visits to referral partners within the assigned territory.
• Represent the facilities at networking events, health fairs, chamber events, and community functions.
• Educate referral sources about the services and specialties offered by each facility.
• Collaborate with Admissions, Clinical Leadership, and Executive Directors to ensure smooth patient transitions.
• Monitor referral activity, admissions, and market trends.
• Develop and execute marketing strategies to increase census and brand awareness.
• Identify opportunities for business growth within the community.
• Maintain accurate records of marketing activities and referral contacts.
• Provide exceptional customer service to referral partners, residents, and families.
• Travel between the Oil City and Saegertown facilities and throughout the surrounding communities.
