Job summary

Sweetwater Care is seeking a Healthcare Community Liaison in Bryan, Ohio to build and nurture relationships with hospitals, physician offices, and other referral partners. You will educate partners on Sweetwater Care's post-acute services and help patients and families connect with the right level of care, ensuring smooth transitions and high-quality outcomes.

Qualifications

• Experience in healthcare sales, business development, or related field.
• Proven ability to build and maintain relationships with hospitals and physician offices.
• Track record of success in territory development and census growth.

Sandstone Post Acute is seeking a highly motivated and results-driven Healthcare Community Liaison to join our team in Bryan, Ohio. As a Healthcare Community Liaison, you will be responsible for building relationships with referral partners, educating them on our services, and helping patients and families connect with the right level of post-acute care.

Job Summary
