Job summary

Quality-Healthcare near Athens, Tennessee seeks an Social Services and Admissions Coordinator to support the Admissions Director, Clinical Liaison, and Marketing Director in growing the facility’s census.

The role manages admissions paperwork, Room Control, and occupancy levels while overseeing the Social Services Department and coordinating medically related services for residents and families. It also involves ensuring HIPAA compliance and smooth transitions after discharge.

Qualifications

• Minimum bachelor’s degree or experience in social work or healthcare admissions.
• LPN experience with Tennessee license (current, unrestricted) preferred.

Responsibilities

• Assist Admissions Director, Clinical Liaison, and Marketing Director in growing the facility's census.
• Manage the admission process including Room Control, timely follow up, and occupancy levels; complete admissions paperwork.
• Oversee development, implementation, supervision, and ongoing evaluation of the Social Services Department.
• Meet with residents and families to assist with medically related services, grievances, or placement needs.
• Complete assessments and participate in comprehensive care plans.
• Facilitate residents' safe discharge and transportation for appointments or events.
• Maintain confidentiality of protected health information in all communications.
