Job summary

Bonaventure Senior Living in Vancouver, WA is seeking a compassionate Health and Wellness Coordinator to support the health services team, onboard residents, and coordinate care with providers.

You will lead training, ensure regulatory compliance, maintain documentation, and communicate with families to align care with resident goals.

Qualifications

• State certifications and licenses are required or obtainable.
• Experience supervising and training staff.
• Knowledge of state regulations and company policies.
• English language proficiency required; CPR preferred; background checks and drug testing are standard.

Responsibilities

• Welcome new residents and assist onboarding.
• Participate in daily stand-up meetings and relay resident concerns to leadership.
• Perform regular quality assurance and resident-chart audits for Assisted Living and Memory Care.
• Review incident reports and coordinate interventions and regulatory reporting.
• Maintain health services documentation, including assessments, service plans, progress notes, and resident records.
• Coordinate service plan updates, provider recommendations, and nursing assessments.
• Manage care coordination with outside providers, admissions, and appointments.
• Communicate with residents' families and facilitate family meetings.
