Qualifications

  • 1–2 years of experience as Health Unit Coordinator, Medical Records Coordinator, Unit Secretary, Medical Secretary, or similar healthcare administrative role required.
  • Experience in long-term care, skilled nursing, assisted living, rehabilitation, hospital, or similar healthcare setting preferred.
  • Working knowledge of medical terminology.

Responsibilities

Health Unit Coordinator-Medical Records

Location: Plymouth, MN (Onsite)

Job Type: Part-time, Non-Exempt (0.5 FTE, 20 hours per week)

Department: Nursing

Reports To: Director of Nursing

Job Summary

Under the direction of the Director of Nursing, the Health Unit Coordinator - Medical Records supports the efficient operation of the nursing department by coordinating administrative functions at the nursing station and maintaining resident medical records in accordance with federal and state regulations, organizational policies, and professional standards. This position serves as a key point of contact for residents, families, visitors, and interdisciplinary team members while helping ensure the accuracy, organization, confidentiality, and timely processing of resident health information.

This is a part-time hourly position, 0.5 FTE (40 hours every two weeks), Monday through Friday. Typical schedule may include occasional evenings and weekends based on operational needs.

Qualifications
  • Associate degree in Health Information Management, Healthcare Administration, Medical Office Administration, Health Unit Coordinator, Medical Secretary, or a related field preferred.
  • 1-2 years of experience as a Health Unit Coordinator, Medical Records Coordinator, Unit Secretary, Medical Secretary, or in a similar healthcare administrative role required.
  • Previous experience in a long-term care, skilled nursing, assisted living, rehabilitation, hospital, or other healthcare setting preferred.
  • Working knowledge of medical terminology.
  • Experience maintaining confidential medical records and handling sensitive resident information.
  • Excellent customer service and interpersonal skills with the ability to communicate effectively and professionally with residents, families, visitors, physicians, nurse practitioners, and team members.
  • Effective organizational, time management, and multitasking skills with the ability to manage competing priorities and frequent interruptions.
  • Demonstrated proficiency with computer systems, electronic health records (EHRs), Microsoft Office applications, and standard office equipment.
  • Professional telephone etiquette and attention to detail.
  • Ability to read, write, speak, and understand English.
  • Must successfully complete a Minnesota Department of Human Services background study and a U.S. Office of Inspector General exclusion search.
Essential Functions
Nursing Unit Coordination
  • Answer telephone calls, relay messages, and direct inquiries to appropriate personnel.
  • Assist residents, families, and visitors by providing information, guidance, and referrals as appropriate.
  • Communicate service requests and needs to other departments, including housekeeping, maintenance, dietary services, and other support departments.
  • Maintain an organized and professional nursing station environment.
  • Order, stock, and maintain nursing unit forms and office supplies.
  • Schedule outpatient appointments and coordinate transportation as needed.
  • Communicate appointment information to residents, families, and responsible parties.
  • Schedule ancillary service visits and obtain required documentation and consents.
Medical Records Management
  • Prepare resident medical records for admissions, readmissions, transfers, and discharges.
  • Process physician, nurse practitioner, and consultant orders in accordance with facility policies and procedures.
  • Ensure orders are communicated and transmitted to appropriate providers, pharmacies, and service partners in a timely manner.
  • Coordinate laboratory, radiology, and other diagnostic service requests as ordered.
  • Prepare documentation for transfers, discharges, and outside appointments.
  • Audit medical records as assigned.
  • Maintain medical records through accurate filing, chart organization, chart thinning, and document retention procedures.
  • Track and maintain physician visit documentation.
  • Request and obtain medical records and supporting documentation as needed.
  • Route documentation requiring signatures or further processing to the appropriate parties.
  • Review physician order renewals and notify supervisory staff regarding outstanding documentation as appropriate.
  • Respond to authorized medical records requests while maintaining confidentiality and compliance with applicable privacy regulations.
  • Maintain resident medical records in accordance with federal and state regulations and professional standards.
General Responsibilities
  • Maintain regular attendance and punctuality in accordance with facility policies.
  • Attend required training, in-services, and meetings.
  • Provide coverage for other nursing units as assigned during vacations, illnesses, or other absences.
  • Maintain professional conduct, appearance, and boundaries with residents, families, visitors, and coworkers.
  • Support facility goals, policies, and departmental objectives.
  • Foster positive working relationships and a collaborative work environment.
  • Perform other duties as assigned.
Physical Requirements and Work Environment
  • Ability to sit, stand, walk, bend, and perform office-related duties for extended periods.
  • Ability to lift, carry, push, or pull up to 30 pounds occasionally.
  • Frequent use of computers, telephones, and office equipment.
  • Potential exposure to communicable diseases and healthcare-related workplace hazards.
  • Must maintain the physical and emotional ability to perform essential job functions safely and effectively.
Compensation:

Pay range is between $21.86 - $23.62 per hourlybased on qualifications, skills, experience, and other relevant factors.
