Mission Nursing Home in Plymouth, MN is seeking a Nurse Aide Registrar (NAR) for on-site, on-call, nonexempt work within the Nursing Department reporting to the Director of Nursing. The NAR provides routine daily care per care plans and reports changes to nursing staff to support resident wellbeing.

Responsibilities include assisting with personal care, documenting vital signs, managing meals, and ensuring a safe, respectful, and confidential environment in line with facility policies and OSHA

Qualifications

  • Completion of approved Nursing Assistant training program.
  • Pass a State of Minnesota criminal background study and OIG exclusion search.
  • Effective oral, written, and reading communication skills.

Job Summary

Under the supervision of the Director of Nursing, the Nursing Assistant Registrar (NAR) provides assigned residents with routine daily nursing carein accordance withestablished nursing procedures and resident care plans. This position also reportsobservedchanges in resident conditions to nursing staff and performs all duties consistent with the facility’s mission and values.

Essential Functions/ Position Responsibilities
  • Assists residents with personal care including bathing, dressing, grooming and toileting with focus on resident-directed care.
  • Make every effort to meet resident's needs by carefully following their written plan of care.
  • Assist with efforts to improve resident outcomes through ambulation training, bowel and bladder retraining programs, range of motion, hydration and nutrition monitoring, pain control efforts and repositioning schedules, and document appropriately.
  • Follows all shift routines, policy and procedures regarding accurate performance of vital signs, weights, etc.
  • Delivers meals and assists residents with eating. Records accurate food and fluid intake as assigned. Report any chewing or swallowing difficulty to the nurse.
  • Assist the resident with the restorative nursing program including but not limited to passive and active range of motion, ambulation with appropriate assistive devices and use of transfer belt.
  • Assists residents in transferring using a transfer belt, device or appropriate lift maintaining resident safety at all times.
  • Reports promptly to the nurse any changes in the resident conditions.
  • Answers call lights promptly and determine resident’s needs.
  • Neatly makes and changes bed linen.
  • Ensures residents are cleaned, neatly dressed, and well-groomed.
  • Provides and communicates any information on changes in skin conditions to the nurse.
  • Observe and report any noted changes in mental status to the nurse.
  • Reports to charge nurse all incidents, accidents that occur involving residents, and visitors.
  • Follows standard precautions, facility safety policies, and work within OSHA guidelines.
  • Attends in-services education and staff meetings as required.
  • Treats and ensures that residents are treated with respect and dignity.
  • Protects organization’s value by keeping patient information confidential.
  • Consistently follows facility’s mission and values.
  • Immediately reports any suspected abuse or neglect per facility policy.
  • Remains aware and takes special precautions to ensure a clean safe environment for residents and staff.
  • Reports any equipment malfunctions to designated employees, including knowledge of Lockout/Tagout Policy and Procedure.
  • Accurate documentation of vital signs, weights, intake and output per facility protocol.
  • Attend designated nursing meetings, as scheduled.
  • Communicates to nurses any changes in resident conditions, complaints of pain and/or mobility changes.
  • Practice good body mechanics to avoid injury. Adhere to minimal lift policy and follow transfer instructions as outlined by the resident’s care plan. Communicate with nursing any changes in residents’ transferring abilities.
  • Assist with Vital Signs, as assigned.
  • Complete required documentation immediately after providing cares.
  • Participates in Unit Quality & Safety Meetings, as directed.
  • Other duties as assigned.
Position Requirements
  • Completion of approved Nursing Assistant training program
  • Successful completion of written and skills exam, as provided from a State of Minnesota approved testing location and be in good standing with Minnesota Nursing Assistant Registry
  • Must be able to pass a State of Minnesota Department of Human Services criminal background study, as well as a United States Office of the Inspector General exclusion search
  • Effective oral, written, and reading communication skills
  • Friendly and professional bedside manners
Physical/Mental Demands

Ability to endure prolonged walking, sitting, standing, use of pulling, bending, and stooping movements. Must be able to lift a minimum of 50 pounds. Frequently pushes or pulls residents 300-600 feet with weight resistance equivalent to 35 pounds. Frequently twists, stoops, bends, squats, kneels. Occasionally lifts and carries equipment and supplies weighting up-to 35 pounds. Frequently reaches above or at shoulder level. Ability to adapt to changes in daily work hours and schedule.

  • Must be able to clearly communicate in English orally and in writing with residents and staff
  • Must be able to read small print in typed and written material
  • Possesses necessary skills to continuously comprehend and communicate with residents, co-workers, and to respond to emergency alarms
  • Identifies residents, call lights, unusual occurrences on unit
  • Concentrates with frequent interruptions and remembers multiple messages, information and task assignments to perform a variety of tasks during an eight hour or longer periods of time

Salary/Pay Range: Wage Scale is between $22.50-$25.10 per hour based on experience.
