Job summary

Santa Cruz Community Health is seeking a Referral Coordinator to facilitate specialty referrals and coordinate care for patients across multiple sites. You will educate teams, advocate for patients, and maintain accurate referral records in the EHR to ensure timely access to specialty services.

The role requires strong communication, teamwork, and bilingual ability in English and Spanish, with in-person clinic work and variable hours. Compensation ranges hourly, DOE, at $27.05–$33.25.

Qualifications

• Two years experience in a healthcare environment or similar setting.
• Strong communication and interpersonal skills with patients and teams.
• Bilingual English/Spanish preferred to support diverse patient population.

Responsibilities

• Receives referrals, completes authorization, scheduling, tracking and follow-up per SCCH policies.
• Schedules urgent/stat specialty appointments and ensures patient information is complete.
• Operates as liaison between patients, PCPs, and specialty offices to ensure timely care.
• Tracks referrals in EHR and communicates progress to care teams.
