Saint Agnes Medical Center is seeking a full-time CVHP Operations role in Fresno, focused on health promotion and chronic disease management. You will advise clients, coordinate with physicians and staff, and guide patients through wellness plans, while addressing Medicare STARS, HEDIS, and HCC requirements.

The ideal candidate has a nursing or health education degree, 3+ years in health promotion, and fluent English/Spanish communication skills.

Qualifikationen

• Bachelor’s degree in Nursing, Health Education, or related field is required.
• Three years of experience in health promotion or chronic illness management is required.
• Knowledge of disease and lifestyle topics such as smoking cessation, weight management, nutrition, pre/post-natal care, stress reduction and chronic conditions is required.
• Proficiency in statistical reporting, spreadsheets, word processing, and databases is required.
• Ability to work independently with strong organizational skills and flexibility.
• Ability to build positive working relationships with a wide range of individuals is required.
• Experience in quality and/or data analysis is required.
• Bilingual fluency in English and Spanish is required.
• Experience with Electronic Health Record preferred.
• Certification from an accredited wellness coach program is preferred.

• Advise and counsel on essential health requirements and healthy lifestyles.
• Identify health risk factors and manage chronic illness with clients.
• Explain Medicare STARS, HEDIS, and HCC to provider staff and close gaps in quality of care.
• Use motivational interviewing and reflective listening to understand barriers to goals.
• Develop client-specific wellness plans and monitor progress toward health goals.
• Coordinate services with other health-related departments for optimal care.
