Job summary

CHI St. Alexius Health is seeking an RN Health Coach to provide individualized care across the life span in an ambulatory setting. You will guide patients through prevention, health maintenance, diagnosis, treatment, and follow-up, while coordinating with specialty practices and inpatient facilities.

You will educate patients and families, set goals, plan actions, and support behavior change. Strong communication, independence, and organizational skills are essential to succeed in this role.

Qualifications

• Must be licensed with the applicable state nursing board and/or have multistate licensure.
• Must hold a valid driver’s license.

Responsibilities

• Utilize the nursing process to manage patients with chronic disease within the Patient-Centered Medical Home.
• Conduct pre-visit chart reviews to identify gaps in preventive health maintenance.
• Collaborate with physicians, APPs, staff, and other professionals to coordinate care across the continuum.
• Facilitate mutual goal setting and goal-directed interactions with the client and family.
