Job summary

American Health Plans seeks a seasoned Program Manager to lead and execute the Participating SNF Engagement strategy, building strong, collaborative relationships with nursing home partners and internal health-plan teams.

You will manage cross-functional projects to enroll facilities, track performance, and deliver solutions that reduce avoidable hospitalizations and lower total costs of care while ensuring compliance with federal requirements.

Qualifications

• 3+ years in health care / managed care with increasing responsibility and ability to lead teams.
• Experience with value-based and risk-based negotiating arrangements.
• Strong ability to persuade external executives and stakeholders.

Responsibilities

• Lead and execute the Participating SNF Engagement strategy across facilities.
• Collaborate cross-functionally with Health Plan, Network, Marketing, Finance and Operations.
• Track KPIs, report progress, and manage implementation documentation.
• Develop mitigation plans to maximize enrollment and engagement.
• Ensure compliance with all applicable laws and corporate policies.
