Reports To: Agency Manager, IHSS Healthcare Professional, Supervisor

Supervises: NONE

The health maintenance attendant shall provide personal and health maintenance care as instructed.

DUTIES AND RESPONSIBILITIES

Essential duties include:

• Provision of health maintenance tasks per care plan and only as instructed for the assigned consumer, which may include traditional CNA level personal care or basic nursing tasks;
• Assisting the client with or providing personal care as assigned by the plan of care, including bathing, hair, skin, mouth care, toileting, dressing, transfer, ambulation, medication reminding;
• Performing light housekeeping duties to keep the client safe, such as making beds, cleaning bathrooms, laundry, kitchen, shopping, and errands;
• Preparing and serving meals;
• Documenting activities, client concerns, and difficulties, reporting to supervisor or manager;
• Providing services as scheduled and communicating schedule changes promptly;
• Completing and submitting paperwork accurately and on time;
• Participating in client case conferences;
• Representing the agency professionally and dressing appropriately;
• Reporting any changes in the consumer to IHSS Healthcare Professional.

Other duties as assigned.

EDUCATION and/or EXPERIENCE

High school diploma or GED accepted. Minimum of two (2) years providing personal care, with six (6) months accepted; preferably one (1) year with licensed agency experience.

ABILITIES, SKILLS AND KNOWLEDGE

Ability to read, write, and understand English; send/receive emails; use computer for communication and documentation; maintain confidentiality; knowledge of home health care practices; effective communication skills; ability to establish and maintain effective relationships.

SPECIAL REQUIREMENTS

• At least 18 years old;
• No history of license or certification suspension or revocation;
• Current TB screening upon hire and annually;
• Valid Colorado Driver's License with reliable transportation or alternative transportation statement.

PHYSICAL REQUIREMENTS

Ability to sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach, talk, hear, finger, handle, feel, climb, use sense of smell, lift/move up to 50 pounds regularly and 100 pounds occasionally, and perform physical assists safely. Vision abilities required include close, distance, peripheral, depth perception, and focus adjustment.

WORK ENVIRONMENT

Frequent exposure to outside weather, variable conditions in private homes, unpredictable lighting, acoustics, air quality, temperature, and hazards. Occasionally exposed to wet/humid conditions, animals, fumes, chemicals. Noise level is usually moderate.

TRAVEL

Frequent travel required for consumer visits.
