Job summary

Community Healthcare Network is seeking a Senior Advanced Practitioner Nurse/Clinician for our Mobile Units to lead clinical operations at a CHN Center and supervise other clinicians.

The ideal candidate holds a New York State license to deliver medical care and has at least one year of post-graduate experience in Primary Care; this role offers a competitive salary and generous benefits.

You will coordinate care with the Center Director to manage services and support underserved communities.

Qualifications

Community Healthcare Network, is a Federally Qualified Health Center (FQHC) serving the underserved communities of Brooklyn, Bronx, Manhattan and Queens. Through the care we provide at our Health Centers and through our various programs, we touch the lives of 85,000 people annually.

We are seeking a Sr. Advanced Practitioner Nurse/Clinician for our Mobile Units. This position is responsible for assuming leadership in clinical affairs at a particular CHN Center; supervises other clinicians at that Center and teams with the Center Director to manage medical operations at the Center.

Essential Functions

• Reviews and interprets medical history data and continually updates this information as necessary.
• Works with Human Resources to periodically review the EHS policies & procedures.
• Oversees the complete and appropriate medical examination on all employees as outlined in the agency’s EHS

The ideal candidate will have a valid N.Y.S. License to deliver medical care and at least one (1) year of post-graduate/residency experience delivering care in a Primary Care setting.
