City of Hope in Irvine, CA is seeking an experienced health information management specialist to ensure electronic health records are processed timely and accurately, in compliance with HIPAA and regulatory standards.

You will support documentation, imaging, and data quality across the HIM workflow, assist with coding and charge capture, and help maintain a complete and auditable record for patient care and revenue cycle operations.

• Three years medical record experience required. Solid knowledge of regulatory requirements governing medical record processing, charge capture, and release of information.
• Knowledge of Title 22, CMS, Joint Commission Standards, State and Federal regulations, including HIPAA relating to record processing requirements and release of information.
• Post High School or equivalent education required.

• Documentation Support/ROI: Review records for completeness and assign deficiencies to clinicians for timely completion.
• Document Imaging/Clinical Records Management/Data Integrity: Scan, index and review documents with adherence to SOPs and SLAs.
• Coding and Data Quality Support: Review accounts, capture charges, and combine records for same-day services.

Position Summary: This experienced health information management specialist performs specific duties in accordance with the HIMS department policies, procedures and standard operating procedures, Medical Staff Bylaws, Rules and Regulations, State and Federal regulations and customer service level agreements to ensure all electronic health records are processed and completed timely and accurately to support COH patient care and revenue cycle operations. Adheres to HIPAA regulations regarding the privacy and security of PHI. Performs HIS 2 duties if and when needed.

As a successful candidate, you will:

Documentation Support/ROI:

• Concurrent and post discharge/visit reviews of medical records for completeness in accordance with medical staff bylaws, rules and regulations, accreditation agencies, state and federal requirements.
• Assigns record deficiencies in the EHR to the appropriate clinician for timely completion.
• Provides support to clinicians with record completion activities, and appropriately responds to provider messages.

Document Imaging/Clinical Records Management/Data Integrity:

• Performs all scanning, indexing and review of documents, adhering to policy, standard operating procedures and service level agreements with clinical departments.
• Performs point of care scanning generated during an inpatient stay to deliver a more timely and complete electronic record.
• Performs quality reviews both prior to releasing to electronic health record and/or retrospective reviews to ensure accuracy and legibility of indexed documents.

Coding and Data Quality Support:

• Performs account review to identify charge errors and or charge movement as necessary.
• Captures charges for clinic infusions, drug administration and extended recovery accounts.
• Combines patient accounts as required to enable coding and billing for services provided on same day.

Your qualifications should include:

• Post High School or equivalent
