Job summary

Highland Park Rehabilitation and Care Center seeks a Medical Records Director (HIM) to ensure accurate, timely, and confidential maintenance of resident health information in line with Colorado regulations and facility policies.

This role supports clinical operations, survey readiness, and health information integrity in our skilled nursing facility, collaborating with the interdisciplinary team while maintaining strict privacy standards.

Qualifications

• High school diploma or equivalent required; HIM coursework preferred.
• Minimum 1 year of experience in skilled nursing or long-term care.
• Proficiency in EHR systems and Microsoft Office.
• Strong organizational, detail-oriented, and deadline-driven skills.
• Excellent verbal and written communication; ability to work independently.

Responsibilities

• Maintain, organize, and secure medical records per CDPH/CMS and facility policies.
• Review charts for accuracy; follow up to resolve deficiencies with staff.
• Ensure documentation meets Colorado survey readiness standards.
• Coordinate release of health information per HIPAA and state laws.
• Prepare reports/logs for state inspections, surveys, and audits.
• Manage record retention and destruction per Colorado schedules.
• Provide timely health information to interdisciplinary team for care decisions.
• Participate in QAPI initiatives related to documentation and accuracy.
• Maintain confidentiality and comply with privacy regulations.
