Job description

Care Management is a service model where all of an individual's caregivers communicate and coordinate to comprehensively address the patient's needs. This role is primarily overseen by a "care manager" who ensures access to necessary services to prevent hospitalizations, maintain health, and promote stability.

Responsibilities of the Care Manager include:
  1. Obtaining enrollment consents from individuals or guardians.
  2. Conducting initial and ongoing needs assessments to determine appropriate care levels.
  3. Managing the patient's Individualized Plan of Care, which involves:
    1. Coordinating service provision according to acuity levels.
    2. Supporting adherence to treatment plans.
    3. Monitoring and evaluating needs across medical, mental health, social, and community services.
  4. Meeting client contact requirements, including face-to-face visits for children as per acuity levels.
  5. Documenting care activities accurately and timely using designated portals and records.
  6. Advocating for clients within and outside the agency.
  7. Linking enrollees with resources and promoting wellness and prevention strategies.
  8. Educating clients and caregivers on managing chronic conditions and preventive care.
  9. Assisting clients in obtaining public benefits like Medicaid, SNAP, housing, and employment supports.
  10. Communicating effectively with clients, families, and caregivers, respecting literacy and cultural preferences.
  11. Participating in care planning meetings and interdisciplinary team activities.
  12. Identifying and managing community resources and referrals.
  13. Engaging in discharge planning for hospital admissions.
  14. Participating in ongoing training to enhance skills.
  15. Ensuring children receive periodic evaluations and follow-ups for dental, vision, and hearing care per Medicaid EPSDT guidelines.
  16. Performing additional duties as assigned by the agency.
Minimum Qualifications:

Candidates should meet at least one of the following educational or credential requirements:

  • Bachelor's degree with two years of relevant experience.
  • Registered Nurse license with two years of relevant experience.
  • Master's Degree in a related field with one year of relevant experience.
  • Bilingual in English/Spanish is a plus.

Additional requirements include a flexible work schedule, a valid New York State driver's license, and physical ability to perform duties including walking, standing, and driving.

Employment Details:

This is a full-time position, 35 hours/week, Monday through Friday, with a flexible schedule based on client needs. Only selected candidates will be contacted for interviews. Benefits include training opportunities, health insurance, paid time off, retirement plans, and more.
