Health Home Care Manager

Care Manager Role

The Health Home Care Manager will help children that have a medical or mental health diagnosis stay safe in their home and communities by connecting them with the right providers and specialists and coordinating with all care team members involved to work collaboratively to ensure the best outcome for the member.

Tasks and Responsibilities

1. Conduct assessments to identify member’s and family’s needs.
2. Create a “plan of care” that will include the needs identified in the assessments and will add appropriate goals and tasks to address the needs.
3. Provide services to help members achieve those goals.
4. Coordinate services.
5. Timely documentation of progress.
6. Meet with member and family monthly.
7. Maintain consistent communication with care team members as needed, including physical, mental, and dental providers, MCO, school teachers, and principals.
8. Keep member profiles in compliance at all times by ensuring that all assessments, progress notes, and other member documentation are up to date.

Requirements

1. Bachelor's degree (120 college credits)
2. 2 years of relevant experience
3. Some experience in coordination
