St. Luke's University Health Network is seeking a Health Coach/Health Educator to support employee wellness programs. You will counsel individuals, assist with care navigation, and deliver education while maintaining detailed program data in the EMR system.

The ideal candidate has two years of health coaching or health education experience, and a clinical license such as RN or RD. MPH/CHES credentials are preferred.

Qualifikationen

• Two+ years health coaching or health education experience.
• Experience serving employee populations preferred.
• Motivational interviewing experience preferred.
• Ability to engage diverse populations.
• Excellent oral and written communication skills.

Aufgaben

• Serves as a health coach for a designated group of employees and/or spouses.
• Initiates contact, assists with care navigation, connects to programs and services.
• Provides individual or group counseling and education to patients when necessary.
• Maintains complete and accurate documentation on all patients to facilitate follow up and program data reporting.
• Proficient in Microsoft Office – Word, Excel, PowerPoint, Access and Publisher.
• Maintains confidentiality and complies with safety and service standards.
