HealthLinc in Elkhart, IN is seeking a Community Health Worker to support the healthcare team by coaching patients, families, and caregivers. You will conduct motivational interviewing to promote self-management and coordinate referrals to community resources.

Responsibilities include building trust with patients, following up on appointments and resources, and serving as a liaison between clinical staff and patients to ensure coordinated care.

Competenze

• High school diploma or equivalent.
• Formal CHW training program completed.
• CCP certificate (HealthLinc will train after hire).
• 1–2 years of patient/community-facing nonprofit experience.

Mansioni

• Establish trusting relationships with patients and families; provide general support and encouragement.
• Provide ongoing follow-up, motivational interviewing and goal setting with patients/families.
• Serve as liaison between clinical staff and patient regarding appointments, referrals and resources.
• Help patients set personal goals and attend appointments.
• Provide referrals to community agencies as appropriate.
• Assist patients in connecting with transportation resources and remind about appointments.
