Job summary

AdventHealth in Sanford, FL is seeking a Medical Assistant to educate members on their care team and support care coordination, including scheduling and referrals. You will assist with front desk inquiries, perform health screens, review records, and help ensure smooth patient visits across the continuum.

The role involves RAF reviews, welcoming patients, and maintaining the clinical space, with duties expanding to check-out and post-visit follow-ups as needed.

Qualifications

• Knowledge of chronic disease management.
• Empathetic nature and ability to provide top-tier service.
• Experience in guest relations or customer service (preferred).
• Team-based care approach and strong English communication.
• Ability to manage care plans under RN guidance.

Responsibilities

• Educates members on their care team and coordinates care, including scheduling appointments and referrals.
• Participates in daily operations of the practice, answering calls and assisting front desk inquiries.
• Performs health screens, reviews records, and identifies care gaps.
• Conducts RAF reviews, greets patients, and reviews welcome packets.
• Manages check-out, follow-up calls, and other duties as assigned.
