Responsibilities

• Resolves patient issues autonomously and serves as the point of contact for escalations.
• Assists with screenings per physician orders and enters data accurately.
• Plans proactively for each patient’s medical needs and follows through.
• Identifies trends, audits, and implements system improvements.
• Provides guidance and motivation to guides to achieve goals.
• Coordinates patient care tasks and ensures adherence to care plans.
• Manages EHR and lab tasks, referrals and results to physicians.
• Resolves order clarification issues and trouble-shoots patient problems.
• Closes the loop with providers after referrals and notes collection.
• Schedules follow-up appointments and closes care gaps in EHR.
• Performs clinical intake according to policy and procedures.
• Performs other duties as assigned.
