Job summary

Centerstone is seeking a dedicated Behavioral Health Coach to join our team in Indiana. You will provide individual coaching, help clients access medical and behavioral health resources, and coordinate care with a network of providers to improve health outcomes.

This role emphasizes compassionate support, documentation, and adherence to CCBHC standards within a multidisciplinary team. Two years of direct service experience preferred; high school diploma required with BA/BS preferred.

Qualifications

• High school diploma or GED required; BA/BS preferred.
• Two years of direct service experience in human services preferred.
• Equivalent work experience may be substituted.

Responsibilities

• Provides individual, supportive coaching to improve physical health status.
• Coordinates client care with a team of internal/external providers.
• Assist clients in accessing resources and improving health self-management.
• Promotes outcomes within the CCBHC model and adheres to standards.
