Job summary

Centerstone is seeking a dedicated professional to join our multidisciplinary team in Behavior & Primary Healthcare Coordination. You will provide individual coaching to improve clients' health, coordinate care with internal and external providers, and help clients access resources to enhance self-management of symptoms.

Responsibilities include scheduling doctor visits, guiding patients in communicating with healthcare providers, and supporting care across the continuum.

Qualifications

• Education: High School diploma or GED; BA/BS preferred.
• Two years of direct service experience in human services.
• Ability to communicate effectively with clients and team members.

Responsibilities

• Provide individual coaching to improve physical health and assist with medical appointments.
• Coordinate care with internal/external providers and obtain Release of Information as needed.
• Offer life skills training and care management to improve access to resources and self-management of symptoms.
