Job summary

Community Bridges, Inc. (CBI) seeks a SSVF - Health Care Case Manager to coordinate medical and behavioral health care for veterans with complex needs in Arizona.

The role liaises with VA, grantees, and community clinics to ensure access to services and delivers care coordination, education, and collaboration across care teams. The position emphasizes adherence to health care plans, timely documentation, and culturally sensitive care for veterans and at-risk populations in the program’s SSVF

Qualifications

• High school diploma or GED required; associate degree in social sciences preferred.
• Minimum of one year of experience in case management, community resource coordination, and interdisciplinary team collaboration.
• Experience with veterans or vulnerable populations strongly preferred.
• Knowledge of available community resources preferred.
• Minimum six months recovery from substance use or mental health disorders preferred.
• Valid AZ driver’s license and clean 39‑month motor vehicle record.
• Behavioral Health Technician certification per A.A.C. R9-10-101.33 and CBI policies.
• Fingerprint Clearance Card required.
• Peer Certification if applicable.

This role supports the Community Bridges Supportive Services for Veteran Families (SSVF) program by coordinating comprehensive medical and behavioral health care for veterans with complex needs.
