Job summary

Fallon Health in Massachusetts seeks Health Aides or CNAs to support Summit Eldercare at PACE, delivering personal care, light housekeeping and ADL support in participant homes and centers. You’ll work under RN supervision, contribute to care plans, and help maintain infection control per CDC guidelines.

We offer $21.50 per hour on weekdays and $24.00 per hour on weekends, mileage reimbursement, and a predictable, low-volume setting focused on quality care with autonomy and flexible scheduling.

Qualifications

• Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but equivalent work experience may be considered.
• One year of experience with frail/older adults preferred, especially in PACE settings.
• Must possess a valid driver’s license and reliable transportation.

Responsibilities

• Assist with ADLs and light housekeeping in participant homes or PACE center.
• Support development and execution of care plans with the Interdisciplinary Team.
• Maintain infection control practices and accurate EMR documentation.
• Provide meal assistance and ensure participant safety and comfort.
