Thriving Together Support Services, LLC in Greenfield, IN is seeking Home and Community Support Professionals (HCSPs) to join our compassionate team. You will provide direct care, assist with daily living activities, and help clients thrive in their daily lives.

Responsibilities include transporting clients to appointments, documenting progress, and collaborating with families and the care team to ensure safety and independence. Shift potential includes evenings and weekends as needed.

Formación

• Experience as DSP/HCSP or direct care role preferred.
• Genuine compassion, patience, and dedication to supporting others.
• Ability to pass background check and state compliance requirements.
• Valid driver’s license and reliable transportation preferred.

• Assist clients with daily living activities including personal hygiene, meal preparation, and mobility.
• Implement individualized support plans to promote independence and quality of life.
• Transport and accompany clients to appointments, community outings, and activities.
• Document care activities, observations, and progress accurately and in a timely manner.
• Communicate effectively with clients, families, and care team members.
• Foster a safe, supportive, and positive environment for all individuals served.

SHIFT: 6 am - 10 am - Monday thru Friday

This is a fast-paced, multiple location shift helping folks get ready for the day, administering medications, and driving individuals to day service. Must have valid driver's license and valid personal care insurance. Will drive company car the majority of shifts.

• Strong communication and interpersonal skills
• Ability to work independently as well as collaboratively with a care team
• Flexibility to work various shifts including evenings, weekends, or holidays as needed
