Thriving Together Support Services in Greenfield, IN is seeking Home and Community Support Professionals (HCSPs) to join the team. This role serves individuals who need support to thrive in daily life, with opportunities for full-time, part-time, and weekend positions.

As part of a compassionate care team, you will provide direct support, assist with daily living activities, and help implement individualized plans to promote independence and quality of life.

Qualifications

• Previous experience in a DSP/HCSP/caregiver or direct care role preferred.
• Genuine compassion, patience, and dedication to helping others.
• Ability to pass a background check and meet state requirements.
• Valid driver's license and reliable transportation preferred.
• Strong communication and interpersonal skills.
• Ability to work both independently and with a care team.

Responsibilities

• Provide direct care and personalized support to individuals with disabilities or special needs.
• Assist clients with daily living activities including personal hygiene, meal preparation, and mobility.
• Implement individualized support plans to promote independence and quality of life.
• Transport and accompany clients to appointments, community outings, and activities.
• Document care activities, observations, and progress accurately and timely.
• Communicate effectively with clients, families, and care team members.
• Foster a safe, supportive environment for all individuals served.
