Job summary

Source Care Group Co is seeking a dedicated Direct Support Professional to join our Indianapolis team. You will provide personalized care to individuals with disabilities, support daily living activities, and help them achieve independence in a compassionate, patient manner.

• Implement individualized support plans (ISPs) and document progress accurately
• Transport and accompany clients to appointments, community activities, and outings
• Administer medications and monitor health conditions per established protocols
• Foster independence and skill development through positive reinforcement and encouragement
• Communicate effectively with supervisors, families, and care team members

Requirements:

• High school diploma or GED required
• Previous experience in direct support, caregiving, or human services preferred
• Valid driver's license and reliable transportation
• Ability to pass a background check and drug screening
• CPR and First Aid certification (or willingness to obtain)
• Strong interpersonal skills with a patient, empathetic, and person-centered approach
• Ability to work flexible shifts including evenings, weekends, and holidays
