Job summary

Doral Health & Wellness is seeking a detail-oriented HCBS Coordinator to support clients enrolled in Home and Community-Based Services. This role focuses on care coordination, Medicaid compliance, documentation, and communication with clients, families, and service providers to ensure high-quality community-based care.

You will coordinate HCBS services, monitor plans, maintain EHR documentation, schedule community services, and assist with Medicaid compliance to ensure services meet regulatory

Qualifications

• High school diploma required; bachelor’s degree preferred.
• Experience in HCBS, Medicaid, case management, or community-based services preferred.
• Strong organizational, communication, and time-management skills.
• Familiarity with EHR or care coordination systems.

Responsibilities

• Coordinate HCBS services and supports for enrolled clients.
• Monitor client progress and ensure individualized service plans are followed.
• Communicate with clients, families, and service providers.
• Maintain accurate documentation in EHR/case management systems.
• Assist with scheduling appointments and community services.
• Support compliance with Medicaid and HCBS regulations.
