Job summary

Doral Health & Wellness is seeking a dedicated HCBS Coordinator to support clients enrolled in Home and Community-Based Services. The role focuses on care coordination, Medicaid compliance, documentation, and clear communication with clients, families, and service providers to ensure high-quality community-based care.

Responsibilities include coordinating HCBS services, monitoring progress, maintaining documentation in EHR systems, scheduling appointments, and ensuring adherence to Medicaid

Qualifications

• HCBS knowledge and Medicaid familiarity.
• Experience with care coordination and documentation is preferred.
• Strong organizational and communication skills required.

About the Role
