Veterans Health Care System of the Ozarks is seeking a Home Based Primary Care Registered Nurse to provide competent, evidence-based care to HBPC patients and supervise licensed practical nurses and home health technicians as appropriate. • Develop and coordinate the patient's plan of care, including assessment, planning, and evaluation.
• Coordinate with the interdisciplinary team to ensure seamless transitions and continuity of care.
