The Harm Reduction Coordinator will deliver in-home harm reduction services and coordinate care for Bridge housing residents across the Bronx, Manhattan, and Brooklyn. Responsibilities include collaborating with internal and external providers, training staff on harm reduction, and prioritizing clients' health, safety, well‑being, and recovery.

Qualifications

• High school diploma or GED is required.
• Experience applying harm reduction with high‑risk populations.
• Proficient computer skills and ability to document services in EHR.
• Spanish bilingual skills are preferred.

Responsibilities

• Provide intensive in-home harm reduction services or referrals for residents in Bridge housing across the Bronx, Manhattan, and Brooklyn.
• Collaborate with internal departments and external providers to coordinate care.
• Offer harm reduction guidance and training to housing staff.
• Promote residents' physical and mental health, safety, well‑being, and recovery.
