The Harm Reduction Coordinator will deliver in-home harm reduction services and coordinate care for Bridge housing residents across the Bronx, Manhattan, and Brooklyn. Responsibilities include collaborating with internal and external providers, training staff on harm reduction, and prioritizing clients' health, safety, well‑being, and recovery.

Qualifications

• High school diploma or GED is required.
• Experience applying harm reduction with high‑risk populations.
• Proficient computer skills and ability to document services in EHR.
• Spanish bilingual skills are preferred.

Scope of Position

The Harm Reduction Coordinator evaluates and addresses substance use-related needs to support residents in thriving within supportive housing.

Essential Position Functions

• Provide intensive in-home harm reduction services and/or referral linkages for a small caseload of residents in Bridge housing throughout the Bronx, Manhattan, and Brooklyn.
• Collaborate with internal departments and external providers to coordinate care.
• Provide harm reduction guidance and training to supportive housing staff.
• Promote clients' physical and mental health, safety, well-being, and recovery.

Qualifications

• Minimum of a high school diploma or equivalent (GED).
• Experience working within a harm reduction model or with high-risk populations.
• Computer literacy required, including the ability to use the agency's electronic health record (EHR) system to document services.
• Bilingual (Spanish) preferred.
