Zusammenfassung

Washington State Department of Social and Health Services is seeking a Habilitation Plan Administrator at Fircrest Residential Habilitation Center in Shoreline to lead Individual Habilitation Plans and guide a multidisciplinary team. The role emphasizes compliance with federal/state rules, resident records, and ongoing care planning.

You will coordinate services across disciplines, monitor progress toward habilitation goals, and communicate with families about plan updates and outcomes.

Qualifikationen

• Meets the professional requirements for a Qualified Developmental Disability Professional (QDDP).
• A degree in Social Work must be from an accredited program.
• Experience requirements depend on degree held (bachelor, master, or doctoral) with relevant disability work.

Aufgaben

• Directs planning, organization, and development of the IDT for Individual Habilitation Plans.
• Integrates, coordinates, and monitors implementation of IHPlans for residents.
• Participates as a QDDP in development of IHPlans on assigned units.
• Monitors progress toward habilitation goals and recommends corrective action.
• Confer with supervisors to evaluate staff contribution toward goals.
• Observes residents and guides staff to ensure proper interactions and behavior management.
• Ensures compliance with Federal and State regulations and maintains unit records.
• Confer with parents/guardians to explain plans and provide progress updates.
• Participates in staff meetings and prepares required reports.
• Evaluates facilities’ programs against DD Administration expectations and reports results.
• Provides advisory assistance for addressing assessed service needs.
• Develops liaison with State/Federal personnel regarding Title XIX service requirements.
