Job summary

Hill Country Mental Health and Developmental Disabilities Centers seeks a Habilitation Coordinator to assess needs of individuals with disabilities and identify outcomes, linking them with services. The role involves coordinating with providers, documenting activity for HHSC compliance and maintaining flexible hours to serve clients who cannot meet 8–5.

Responsibilities include monthly face-to-face contact, creating Habilitation Service Plans, and complying with TAC Title 26, Chapter 33.

Qualifications

• Bachelor's degree from an accredited college or university in social, behavioral, or human services.
• Minimum 1 year of experience working with individuals with developmental disabilities.
• Valid Texas state Driver's License and auto insurance.
• Complete and maintain CPR certification and Prevention and Management of Aggressive Behavior training within 3 months.

Responsibilities

• Respond to all PL1 alerts within required timeframes.
• Complete all PASRR Evaluations accurately and within required timeframes.
• Ensure all individuals on caseload receive face-to-face contact monthly.
• Assess/Evaluate designated residents for eligibility.
• Develop and update person-centered Habilitation Service Plans (HSPs).
• Collaborate with Nursing Facilities and service providers to link individuals with outcome-focused services.
• Meet all timeframes set forth by HHSC regulations.

Job description

The Habilitation Coordinator is responsible for assessing, evaluating, and identifying the needs and goals of people with intellectual and developmental disabilities relating to personal outcomes, including recommendations for interventions and support services. Arranges and collaborates with service providers and agencies to link people with outcome-focused services. Responsible for making contact with assigned individuals in accordance with the person's plan, and accurate and timely documentation that supports outcome-based services. The Habilitation Coordinator is responsible for providing activities in accordance with state and federal rules. Requires flexibility in schedule and some travel. All services are delivered in compliance with Health and Human Services (HHSC) and TAC Title 26, Chapter 33. Office hours are 8:00 am - 5:00 pm; however, the position must have flexible hours to accommodate individuals who cannot meet between 8 and 5.

Essential Duties and Responsibilities

You’ll definitely need

• Bachelor's degree from an accredited college or university in social, behavioral, or human services with the ability to be credentialed as a Qualified Mental Health Professional (QMHP-CS)
• Minimum 1 year of experienceworking with individuals with developmental disabilities.
• Maintain valid Texas state Driver's License and auto insurance.
• Complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training within the first 3 months of employment
