Job summary

Bluebonnet Trails Community Services seeks a Habilitation Coordinator to support PASRR services. You will handle intake, assess needs, coordinate services, and advocate for community living options for adults with IDD and those at risk of admission to nursing facilities.

You will coordinate across systems, support transitions between nursing facilities and community settings, and promote independence with vulnerable adults in Guadalupe, Gonzales, and Caldwell Counties.

Qualifications

• Bachelor's degree in a human services field is required.
• If degree is not in a human services field, minimum of 24 college credits in a Human Services area required.

Bluebonnet Trails Community Services is seeking a passionate, self-motivated Habilitation Coordinator to join our team. This rewarding position serves as a vital advocate for adults with intellectual and developmental disabilities (IDD) and suspected mental illness who reside in, or are at risk of admission to, nursing facilities.

Hourly Salary Range: $ 21.46 - $28.71

• $0.75/hour Bilingual
• $1.00/hour Associate or Advanced licensure
• Up to $4.00/hour - Tenure with BTCS
• $0.50/hour - LCDC

Credentials

Credentials Minimum Qualifications:

• A bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, or early childhood education/intervention.
• *If degree is not in a human services field, a minimum of 24 college credits in a Human Services are required.
