Job summary

You will provide instruction in social living skills, supportive counseling, medication management, ISP development, and coordination with families, physicians and staff, while maintaining records and abiding

Qualifications

• Thorough knowledge of care for individuals with intellectual disabilities.
• Knowledge of service plans, case management, and ISPs.
• Ability to maintain records and reports and to use electronic records.

Responsibilities

• Provides instruction and training in social living skills.
• Provides supportive counseling and crisis intervention.
• Links residents with services and assists with appointments.
• Transports residents and coordinates with staff and pharmacies.
• Manages medication administration and related records.
• Maintains required charts, forms and reports.
• Assists in ISP development and monitoring.
