Overview

Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines

Overview

Assists the Manager and provides direct care in the operation of a Supervised Apartment Program or Group Home providing for the medical, physical, psychological and social needs and personal goals for individuals having intellectual and developmental disabilities on a 24/7 basis. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines

Duties And Responsibilities
  • Provides instruction and training in social living skills.
  • Provides supportive counseling.
  • Links resident with appropriate services and assists them in scheduling and keeping appointments.
  • Transports residents.
  • Conducts group meetings with residents.
  • Supervises the self-administration, or administers medication, and prepares and maintains related records, monitors resident medication management and coordinates with staff, prescribing physicians and pharmacies.
  • Records relevant progress notes, completes and maintains required charts, forms and reports.
  • Assists residents with personal financial transactions and record keeping.
  • Plans facility menus and shops for groceries.
  • Coordinates social and recreational programs and activities.
  • Assesses resident needs and assists in the development, monitoring and updating of Individual Service Plans, implements specified services, prepares quarterly updates related to the ISPs.
  • Provides overnight supervision to residents; responding to needs as necessary.
  • Conducts fire drills, inspects smoke detectors and fire extinguishers according to policy, and maintains appropriate records.
  • Communicates with family members, and other appropriate persons.
  • Assists in the upkeep of the house, grounds and vehicles.
  • Provides First Aid and CPR as needed.
  • Assists in the scheduling, training, leading and evaluating the work of residential staff.
  • Assist in monitoring compliance to state and local licensure standards.
  • Enters documentation into the electronic records system.
  • Provides crisis intervention.
  • Inputs service and other documentation to the electronic records system.
  • Completes and maintains required charts, forms and reports.
Knowledge, Skills, And Abilities

Thorough knowledge of the theories, principles, practices and techniques related to the care and treatment of individuals with intellectual disabilities; thorough knowledge of the common disabilities of the individual population; thorough knowledge of human rights of residents; thorough knowledge of services and programs available to the residents within the agency and by external service providers; thorough knowledge of liability issues.

Considerable knowledge of service plan development; considerable knowledge of case management principles and practices; considerable knowledge of the methods and use of electronic health records; considerable knowledge of the maintenance, security and safety practices and procedures related to the operation of a residential facility; considerable knowledge of state licensure requirements.

Ability to establish effective working relationships with a variety of others; ability to communicate effectively orally and in writing; ability to operate a vehicle; ability to collect and analyze data from a variety of sources including electronically and prepare reports; ability to plan, train and supervise the work of others; ability to function calmly in stressful situations; ability to work with individuals and family members with challenging behaviors; ability to interact positively and respectfully with clients and their families; ability to conduct effective interviews, observe, assess and record individual behaviors and functioning and develop treatment and service plans.

Typical Education, Training And Work Experience

Bachelor's Degree in the Behavioral Sciences, and considerable experience in residential treatment programs for intellectually disabled/developmentally disabled adults.

LICENSURE AND CERTIFICATIONS

Valid Virginia driver's license. Certification in First Aid/CPR. Training in Therapeutic Options (THEROPS) and Medication Management.
