Job summary

Sacramento Native American Health Center is seeking a Community Health Program Associate to support grant programs focused on planning, developing, improving, and delivering mental health and suicide prevention activities for Native American youth and their families.

Working with the CH Program Manager and Coordinator, you will assist in implementing grant-funded work plans, data collection/reporting, and coordinating community health activities.

Qualifications

  • Experience helping with community and/or grant funded programs.
  • Must be able to lift 40 lbs and be responsible for set up/breakdown of outreach booth and equipment.
  • Must have a valid California Driver’s License with current auto insurance and reliable transportation.
  • Successfully pass a background check to be permitted to work with children.
  • Must be able to work nights and weekends as needed.

Responsibilities

  • Supports the establishment and implementation of age-appropriate, culturally responsive programs, groups, activities, health education classes, and interventions related to mental health and suicide prevention.
  • With guidance from the Program Manager and Coordinator, helps implement grant funded prevention services and activities including reporting and evaluation activities.
  • Leads belonging activities and discussion groups within Community Health programs, supporting the coordination and facilitation of program content led by the Program Coordinator.
  • Supports onsite staffing of SNAHC Gathering of Native Americans (GONA) and other community health events.
  • Participates in events such as health fairs, pow wows, conferences, and trainings.
  • Assists with set up and break down of outreach booths at community health events, as well as outreach during events and other partnership opportunities.
  • Ensures SNAHC services and programs are delivered in a culturally appropriate manner.
  • Participate in internal quality improvement teams. Works with team members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals for the organization, federal and state laws and regulations, and accreditation standards.
  • Complies with all state and federal laws and regulations, as they pertain to position including; HIPAA, sexual harassment, scope of practice, OSHA, etc.
  • Other duties as assigned.
