Job summary

Native American Health Center in Oakland is seeking a Program Manager I, Home Visiting to lead the Community Wellness Department’s Home Visiting and CPSP initiatives. Qualifications

Bachelor’s degree in public health, social work, or equivalent experience.
2–4 years work experience in community health or nonprofit settings.
Experience with program management and grant administration.
Knowledge of Bay Area Native American community preferred.

Responsibilities

Provide project direction, budget oversight, and evaluation support.
Oversee 40% clinical hours per week with care coordination and outreach activities.
Utilize EPIC to ensure records are complete and documented in the EHR.
Coordinate program staff, volunteers, and interns across multiple programs.
Ensure grant objectives and reporting meet funder requirements.
