Job summary

City of Laredo is seeking a Ryan White Case Manager to develop and implement care plans, engage clients and families, and facilitate access to medical and supportive services. The role prioritizes confidentiality, advocacy, and liaison with health care teams to address barriers to care.

Responsibilities include HIV testing, risk reduction counseling, and generating program reports, with travel to client sites as needed. This is grant-funded and contingent on funds availability.

Qualifications

• Bachelor's degree in Social Work or related field required.
• At least three years of case management or related experience preferred.
• Experience with HIV/AIDS services and linkages is advantageous.

Responsibilities

• Develop and implement health care service plans with client and family involvement.
• Provide HIV/AIDS testing and risk education in clinic and community settings.
• Conduct assessments using standardized tools and document progress.
• Advise and assist with accessing medical and supportive services.
• Maintain confidentiality and prepare narrative monthly activity reports.
