Job summary

Qualifications
- Graduate of an accredited nursing school.
- Current Connecticut RN license.
- BLS certification required.
Responsibilities
- Provides direct nursing care in accordance with policies and protocols.
- Implements and monitors patient care plans and communicates patient condition.
- Acts as primary coordinator of multidisciplinary care.
- Coordinates discharge planning with the healthcare team.
- Assesss and delivers individualized care to meet patient needs.
- Delegates tasks to nursing staff as appropriate.
- Participates in daily interdisciplinary rounds and provides input.
- Adheres to ANA standards, ethics and policies.
- Develops and revises individualized plans of care.
- Supports professional growth and development.
- Performs other duties as assigned.
